ABSTRACT

Background: Neuronal studies on diabetes mellitus (DM) were previously based on peripheral and autonomic nerves. With the advent of Brainstem Auditory Evoked Potential (BAEPs), studies on sensory pathways in the central nervous system become easier and more productive. BAEPs is a non-invasive electrophysiological tool to detect retro-cochlear lesion. Hence, it is helpful to detect early Impairment of the auditory nerve and brainstem function. Aim and Objective: DM Type 2 (T2DM) is a known cause of neuropathy and in earlier course, it involves sensory nerves. T2DM runs in families and it has a genetic predisposition. BAEP is one of the methods to find out problems related to hearing by analyzing latencies of waveforms and inter-peak latencies. Hence, BAEPs in apparently healthy subjects with and without family history of T2DM is assessed. Materials and methods: This is a cross-sectional, casecontrol study. We have taken 110 volunteers from MBBS students of IMS, BHU of 17–23 years of age. Those with co-morbid conditions (eg. diabetes and hypertension), neurodegenerative diseases, neuropathy, schizophrenia, and those on ototoxic and neurotoxic drugs are excluded from the study. After taking consent and conducting a preliminary physical examination, BAEPs are recorded using a proper BAEP recording device. Statistical analysis is done using SPSS 2016 software trial version with Chi-square test. Results: The subjects with positive family history of T2DM in paternal grandfather showed deviation in latencies of BAEPs wave I (P < 0.001), wave III (P = 0.019), wave V (P = 0.033), and inter-peak latency between wave I and wave V (P = 0.019) from the normal values in the left ear. The subjects with positive family history of T2DM in paternal grandmother showed deviation from the normal in case of V/I % in the right ear (P = 0.016). Conclusion: The presence of T2DM in families can affect the wave latencies and inter-peak latencies of BAEPs.

ABSTRACT

Background: Even though India was the first country to launch family planning programme, population growth rate of India is still higher. Males are often the dominant decision makers of the family. Many studies regarding contraception has been done on females but the same is lacking on males. Aims & Objectives: The aim was to assess the level of awareness and practices about the contraceptive methods and factors associated with awareness and use among the married males in slums of Phulwarisharif block of Patna. Material & Methods: A cross-sectional study was conducted among 212 married males aged 18 to 60 years over a period of 3 months in slums of Phulwarisharif, Patna. Data were collected by interview of study subjects using predesigned and pretested questionnaire. Descriptive statistics and chi-square test were applied using SPSS software. Results: Among 212 married males 91.5 % have heard of at least one contraceptive method, while only 42.45% have ever used any contraceptive method. The major source of knowledge among 194 aware participants were friends and family members (78.9%). Age, duration of marriage, number of children and position in the family were associated with level of awareness. Conclusion: Despite of high awareness about contraceptives low utilization and limited knowledge was observed among the married male participants.

ABSTRACT

Background: The impact of neoadjuvant chemotherapy (NACT) on tumour biomarkers and the histopathological response to treatment in breast cancer specimens remains controversial. Chemotherapy and hormonal therapy decisions for breast cancer management are influenced by the expression of tumour biomarkers: estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth receptor 2 (HER2). On the other hand, pathological response is an indicator of chemotherapy effectiveness. The study of the effect of NACT in breast cancer is an important issue.Objectives: To assess the changes to biological markers ER, PR, and HER2, and the pathological response in locally advanced breast cancer patients after neoadjuvant chemotherapy. Methods: 100 patients with locally advanced breast cancer were assessed with core needle biopsy for biological markers (ER, PR, HER2) and pathological grading. Subsequently they were treated with six cycles of taxane based NACT followed by surgical resection. Biological markers and the pathological response (assessed by the Miller Payne grading system) were re-evaluated to assess changes.Results: The patient mean age was 45.62 ± 7.12 years. Most patients (56%) were postmenopausal. Clinical disease stage ranged between any T N2, T3 N1-2, T4 N0-2. Post NACT, pathological complete response rate was 14%, ER positivity decreased from 80 (80%) to 78 (78%) (p = 0.67). PR positive dropped from 66 to 62% (p = 0.002), and HER2 receptor positivity was increased from 22% to 28% (p-value 0.000).Conclusion: It was observed that biological markers (ER, PR, HER2) and the histopathological response of breast cancer may change after NACT. This change may affect treatment decisions
